Terminating Membership (PPO)

- The Health Plan Member Services at 1.877.847.7907 (TTY: 711) - Hours of Operation are October 1 through March 31: 8:00 am to 8:00 pm, 7 days a week and April 1 through September 30: 8:00 am to 8:00 pm, Monday through Friday.
- Medicare at 1.800.MEDICARE (1.800.633.4227), 24 hours a day, 7 days a week. TTY users should call 1.877.486.2048

Potential for Contract Termination
The Health Plan has contracts with Medicare and our provider networks to provide contracted services to eligible members in our service area. If any contract between Medicare or the provider organization is altered for any reason, you will be notified of the change in a timely manner in accordance with Medicare regulations and provided with information on steps you should take to continue to receive Medicare services.
